    Orbital Metastasis from an Occult Breast Carcinoma (T0, N1, M1)

    The authors report a case of an orbital metastasis from an occult breast carcinoma. A 66-year-old woman presented with a growing left orbital tumour. Orbital CT scan was consistent with lymphoma. However, ocular pathology revealed small neoplastic cells showing an 'indian file pattern' suggestive of metastatic carcinoma and immunohistochemistry was positive for CK7, CK CAM5.2 and oestrogen receptor. A systemic evaluation was then performed with mammogram, breast ultrasound and MRI considered normal. An exhaustive systemic evaluation revealed multiple bone lesions, a right axillary lymph node lesion, which presented the same pattern on pathology and immunohistochemistry, with no evidence of a primary tumour. A diagnosis of a metastatic lobular carcinoma of the breast (T0, N1, M1) was made and the patient was started on chemotherapy and adjuvant hormonal therapy.info:eu-repo/semantics/publishedVersio

    IgG4 related disease (IgG4 RD) is a newly recognized entity that includes a group of immune-mediated fibro-inflammatory conditions. It can affect a wide range of organs, producing a broad spectrum of presentations. The selection of imaging modality is based on patient symptoms and the organs under evaluation. The diagnosis is based on a combination of radiological findings, serum IgG4 elevation levels, histopathological features, association with other IgG4-related diseases and response to steroids. We report a rare case of an IgG4-related sclerosing mesenteritis in a man who presented with an incidental right lower quadrant mesenteric mass and was unsuccessfully treated with corticoids.     Good appearance and shape descriptors for object category recognition

    In the problem of object category recognition, we have studied different families of descriptors exploiting RGB and 3D information. Furthermore, we have proven practically that 3D shape-based descriptors are more suitable for this type of recognition due to low shape intra-class variance, as opposed to image texture-based. In addition, we have also shown how an efficient Naive Bayes Nearest Neighbor (NBNN) classifier can scale to a large hierarchical RGB-D Object Dataset [2] and achieve, with a single descriptor type, an accuracy close to state-of-art learning based approaches using combined descriptors

    The Role of Ophthalmic Imaging in Central Nervous System Degeneration in Systemic Lupus Erythematosus

    Systemic lupus erythematosus (SLE) is an autoimmune connective tissue disorder that can involve any organ system. Central nervous system involvement can be a severe life threatening complication, ultimately resulting in severe neurodegenerative changes. Magnetic resonance imaging suggests that neurodegeneration, which may have deleterious effects on brain function, may occur early in SLE and experimental models suggest that neuroprotection may be feasible and beneficial. The retina is an extension of the brain. Recent ophthalmic imaging technologies are capable of identifying early changes in retinal and choroidal morphology and circulation that may reflect CNS degeneration. However, their utility in monitoring CNS involvement in SLE has been poorly studied as these have only been performed in small cohorts, in a cross-sectional design, non-quantitatively and without correlation to disease activity. The authors aim to review the current understanding of neurodegeneration associated with SLE, with particular focus on the visual pathway. We describe the neuropathology of the visual system in SLE and the evidence for retinal and choroidal neurodegenerative and microvascular changes using optical coherence tomography technology.     Direct Percutaneous Endoscopic Jejunostomy Using Single-Balloon Enteroscopy without Fluoroscopy: a Case Series

    BACKGROUND: Direct percutaneous endoscopic jejunostomy (DPEJ) is a useful method to provide enteral nutrition to individuals when gastric feeding is not possible or contraindicated. The aim of this study was to analyze the efficacy and safety of DPEJ tube placement with the Gauderer-Ponsky technique by the pull method, using single-balloon enteroscopy (SBE) without fluoroscopy. METHODS: This is a retrospective analysis of patients undergoing SBE for DPEJ placement in a referral hospital between January 2010 and March 2016. Technical success, clinical success and procedure related complications were recorded. RESULTS: Twenty-three patients were included (17 males, median age 71 years, range 37-93 years). The most frequent indications for DPEJ were gastroesophageal cancer (n = 10) and neurological disease (n = 8). Eighty-seven percent of the patients had a contraindication to percutaneous endoscopic gastrostomy (PEG) and PEG was unsuccessful in the remaining patients. The technical success rate was 83% (19/23), transillumination was not possible in three patients and an accidental exteriorization of the bumper resulting in a jejunal perforation occurred in one patient. The clinical success was 100% (19/19). The median follow-up was five months (range 1-35 months). Apart from the case of jejunal perforation and the two cases of accidental exteriorization, there were no other complications during follow-up. The 6-month survival was 65.8% and the 1-year survival was 49.3%. CONCLUSION: DPEJ can be carried out successfully via SBE without fluoroscopy with a low rate of significant adverse events. Although, leaving the overtube in place during the bumper pulling can be useful for distal jejunal loops, it can be safely removed in proximal loops to minimize complications.info:eu-repo/semantics/publishedVersio
